Please call our office staff to schedule an appointment.

• Please notify your clinician when canceling an appointment at least 24 hours in advance. 

• A $140.00 cancellation fee will be applied to missed or canceled appointments without 24 hours in advance notice.                      

• Clients who miss their initial appointment will incur a $140.00 fee and services will be terminated.

• Ongoing clients with good attendance may be allowed one missed appointment with documentation from a physician. 

Once  you have been contacted by our intake department and assigned to a therapist, you will receive an email from noreply@pimsyprix.com with your login information.*

Child-

• Parental Consent for Treatment of a Minor

All fields must be filled in to move forward on the form, and signatures completed before selecting  “save” at the end of the form.

A Copy of Driver's License or State Issued Identification Card and Insurance Card are also needed.

Please upload a copy of the front and back of your driver’s license or state-issued identification card and a copy of the front and back of your insurance card under the Documents tab>Upload>Select Files>Drop files here to upload. You can also email it to klfrontoffice@karaleeandassociates.com or bring it to the office.
